COORDINATE is the values-led Primary Health Network for South Eastern NSW and is one of 31 PHNs across Australia and one of 10 PHNs in NSW. Our region stretches from Helensburgh in the north, past Eden to the Victorian border in the south, and inland to Cooma/Monaro, Queanbeyan, Yass, and Goulburn.
Together with the diverse government, non-profit, education and research, and private healthcare organizations, we work to transform the health system to improve health outcomes across this unique region. The Engagement and Coordination team is responsible for working with stakeholders in innovative ways to facilitate and integrate service delivery to improve the patients’ journey through the health system and improve health outcomes.
The team is responsible for the provision of practice support, system redesign and other health system improvement projects, pathway optimization, coordination of care, stakeholder alliances, and communications.
The Role
Reporting to the Manager, Primary Care Development, this exciting and influential role is the primary conduit for supporting strategy and change implementations, aimed at improving the health service system. As a Health Coordination Consultant, you would be highly skilled in relationship management and be responsible for developing meaningful relationships and engaging with key stakeholders including, general practitioners (GPs) in your local area.
In doing this, the role of a relationship manager and change agent would be to support and implement strategies and initiatives that are aimed at supporting and/or influencing stakeholders to make quality and system improvements for better health outcomes in our region.
