Job Description

Within the Christchurch & Southbourne Community Mental Health Team, an opportunity has arisen for a Band 6 RMHN or Mental Health Practitioner to work five days per week. This post is initially fixed for 12 months. The post holder will work as part of an integrated multi-disciplinary team.

The team is made up of Psychiatrists, Nurses, Psychologists, Employment Specialists, Physical Health workers, Social Workers, Admin, and Support Time Recovery workers. This is primarily a care-coordination role, where you will be responsible for supporting patients on their journey of recovery.

You will be a committed, dynamic, motivated professional with excellent communication, organizational skills, and the ability to prioritize your work. You should have recent experience working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with service users.

As a key member of the team, you will find the experience challenging, but rewarding and varied within the well-established and supportive team in Christchurch & Southbourne CMHT. To provide a comprehensive accessible and responsive mental health service to Adults and their families within a defined locality. To support the team and locality leader in the implementation of the DHUFT Mental Health Strategy. To continually develop clinical, team, and service evaluation and delivery of service.

To contribute to effective multi-disciplinary working within the Community Mental Health Service and to build up effective liaisons with community, primary, other secondary, and tertiary services and agencies within the locality to provide holistic care packages based on individual needs and service development.

The team is based close to Christchurch Hospital and BCP Council Locality Office; the post holder will need to have the ability to travel by car as they will be required to assess and monitor patients in their own homes and within other communities’ settings.
