Job Description
SOCIAL WORKER – EXTENDICARE VISTA PARK LODGE
Extendicare is a leading provider of care and services for seniors throughout Canada. Applicants are invited to apply for the above full-time position to work at Vista Park Lodge, a 100-bed long-term care facility, located in Winnipeg, Manitoba.
Reporting to the Administrator, the Social Worker provides social work services for residents, families, and staff.
RESPONSIBILITIES
- Provides a complete resident admission process including tours, admission, discharge, and occupancy tracking.
- Acts as liaison as appropriate, with families. Provide counseling or group sessions for residents and families as identified.
- Maintains established department plans, policies, and procedures to meet regulatory and Extendicare standards, including quality assurance.
- Manages the quality care programs including Guardian Angel, Health Tracking, Behaviour Management, Ethics, and Palliative Care.
- Participates in providing in-service programs for residents and families as needed.
- Maintains all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures, and legislative requirements.
- Acts in an advocacy role as appropriate.
- Satisfy Health records as they pertain to the department (MDS experience is an asset)
- Liaises with government officials, Extendicare consultants, and other professionals in relation to resident care needs and department activities.
QUALIFICATIONS
- A Master’s Degree in Social Work from an accredited University is preferred.
- Previous experience in a long-term care setting.
- Knowledge of applicable legislation and regulations.
- Competency in conducting individual counseling and group sessions.
- Computer literate
